Competition terms&conditions

Dear Customer, see below the Terms and Conditions for 5Th Avenue Medical Clinic Xmas Spectacular Competition – chance to win Xmas dinner for two at Sky Garden Restaurant!
Please ensure that you have read and fully understood them prior to entering our competition.

General Terms and Condition
The prize draw is open to all UK residents aged 18 or over except employees of any company or organisation associated with the prize draw.
All entries must be made via 5th Avenue Medical Clinic (over the phone or personally in the Clinic).
To enter the competition, you must spend min 150£ for any treatment available at 5th Avenue Medical Clinic. From all entries, we will draw 1 person who will receive voucher for dinner for two at Darwin Brasserie at Sky Garden London.
The Competition begins at 9am on 15th September (the “Start Date”).
The Competition closes at 4pm on 1st December (the “End Date”).
Winner will be announced on Monday 3rd December 2018.
The winner will be notified by direct message or email within 24hours of the closing date. If the winner cannot be contacted or does not claim the prize within 14 days of notification, we reserve the right to withdraw the prize from the winner and pick a replacement winner.
5th Avenue Clinic will notify the winner when and where the prize can be collected.
5th Avenue Medical Clinic accepts no responsibility for any damage, loss, liabilities, injury or disappointment incurred or suffered by entrants as a result of entering the competition or accepting the prize. Promoter further disclaims liability for any injury or damage to entrant’s or any other person’s computer relating to or resulting from participation in or downloading or uploading any materials in connection with the Competition.
We reserve the right to amend the end date at any time.
All entry instructions form part of the terms and conditions, which, by taking part, all entrants will be deemed to have accepted.
5th Avenue Medical Clinic reserves the right to exclude any Entry from the Competition at any time if they have a reason to believe that the entrant has breached any of the Rules.
By entering the competition, the winner agrees to participate in promotional activity where required.
Prizes are non transferrable to another person.
No cash alternative available.
Travel and expenses are not included unless otherwise stated.
By entering competitions you agree to be contacted for promotional purposes.
No personal data will be shared with third parties unless stated otherwise.

Prior to your appointment we will inform you of any preparation required in advance of your treatment.
If you are unable to attend your appointment, we require 48 hours’ notice in order to reallocate your appointment to another client. Failure to give the required notice may result in your being charged a cancellation fee.
Late arrival may result in reduced treatment time or forfeiting of the appointment. Our Therapists will endeavour to ensure that your appointment runs to time; however, for reasons beyond our control, we may need to cancel or postpone your appointment at short notice. In this unlikely event, we will make every effort to contact you in advance.
It is the client’s responsibility to ensure that he or she provide 5Th Avenue Medical Clinic Therapists with all relevant medical details prior to each treatment. 5Th Avenue Medical Clinic will not be liable for any damage that occurs as a result of the client’s failure to disclose such details.
The customer agrees to comply with all instructions and/or recommendations given to them by, or on behalf of, The 5Th Avenue Medical Clinic regarding the care of a treated area.
